Persons wishing to register are required to provide proof of identity as detailed in the above box. If you are from abroad, please provide proof of your legal status to reside in the UK (either a home office document or visa status stamped in your passport). We will then invite you to make an appointment to see one of our nurses for a new patient medical health check to enable us to gather more basic information about your health. When you register with us, you are registering with the practice not with a specific doctor. When your preferred doctor is not available to see you, we will provide you with an explanation and offer an appointment with another clinician.
